Data protection declaration for the use of forms

I have taken note of the following obligations, to be fully informed about the use of the data provided via the prescription and request forms:
all personal data may be stored and processed on the server of the MHH data centre in accordance with data protection regulations, in particular the GDPR. The employees of the Cellular Therapy Centre of the MHH, who are performing the manufacturing activities specified in the above-mentioned forms, have access to the data.

1. Purpose of data collection
   The personal data collected are used exclusively for the implementation of the production of cellular or other medicinal products specified in the above-mentioned forms and are used and archived in accordance with the provisions of the Medicinal Products Act (Arzneimittelgesetz - AMG).
    
2. Passing on the data
   The collected personal data are generally not passed on to third parties. If necessary, they are passed on within the MHH exclusively to those areas and persons who need this data to fulfil the contractual and legal obligations or to implement the legitimate interests of the MHH.
    
3. Storage
   After collection, the personal data are retained or stored by the Cellular Therapy Centre for as long as necessary to fulfil the prescription. As a rule, this means that the personal data are deleted or anonymized, taking into account the existing storage obligations and periods under medicinal product law.
    
4. Revocation
   In the event of a revocation, the personal data provided via the above-mentioned forms will be deleted, in case that this deletion does not contradict the existing storage obligations and periods under medicinal product law.
    
5. Right
   In the data protection declaration linked under the form, I have taken note of the rights with regard to personal data according to the GDPR.
